On March 13, 2023, the Office of the Chief Coronor implemented a new form, the “Resident Death Notice”. This replaces the previous “Institutional Patient Death Record”. The notice is completed by staff when a resident passes away. It is different than a death certificate, which is completed by a doctor and is sent to the funeral home.
This new “Resident Death Notice” is more comprehensive than the previous record. Many of the questions may require staff to call the coroner to review the resident’s profile. This may lead to the coroner calling resident’s families to discuss the resident’s profile. It is to be expected that more families may be contacted by the coroner – this is part of the new process of completing the “Resident Death Notice”.
